The Prophet (ﷺ) prayed two rak`at of Zuhr prayer (instead of four) and he was told that he had prayed two rak`at only. Then he prayed two more rak`at and finished them with the Taslim followed by two prostrations.

"The Prophet (S) prayed five (Rakah) for Zuhr, so it was said to him: 'Has the prayer been added to, or have you merely forgotten?' So he (S) performed two prostrations after he had said the Salam."

The Messenger of Allah (ﷺ) prayed Zuhr with two rak'ahs, then said the salam. They said: "Has the prayer been shortened?" So he stood up and prayed two rak'ahs, then he said the salam, then he prostrated twice.

"The Prophet(ﷺ) prayed Zuhr with five rak'ahs, and it was said to him: 'Has something been added to the prayer?' He said: 'Why are you asking?' They said: 'You prayed five.' So he turned around and prostrated twice."

“(Once) the Prophet (ﷺ) prayed Zuhr with five Rak’ah, and it was said to him: ‘Has something been added to the prayer?’ He said: ‘What is that?’ They told him, and he turned back towards the Qiblah and performed two prostrations.”

The Prophet (ﷺ) prayed Zuhr with five rak'ahs. It was said: Has something been added to the prayer? And it was said: You prayed five (rak'ahs). And he prostrated twice.

The Prophet (ﷺ) prayed Zuhr or ‘Asr with five rak’ahs, then he did the two prostrations of forgetfulness. Then the Messenger of Allah (ﷺ) said: "These two prostrations are for anyone among you who thinks that he did more or less."
