٥٤١٦ - الحكيم الموفق أبو يوسف يعقوب بن غنايم الشامري الأصل الدمشقي (٥)، المولد والمنشأ، المتوفى سنة [إحدى وثمانين وستمائة]. كان جامعًا للعلوم الحكمية، متقنًا في صناعة الطب، رئيس زمانه وعلّامة أوانه. اشتغل عليه جماعة من المتطببين وصنف تصانيف قوية المباني بليغة المعاني، كـ "شرح الكليات" من كتاب "القانون" وكتاب "المدخل إلى علم المنطق والإلهي". ذكره صاحب "عيون الأنباء" ولم يؤرخ.
