محمد بن هاشم بن القاسم بن هاشم بن عبد الوهاب بن محمد بن إبراهيم بن محمد بن علي بن عبد الله بن العباس بن عبد المطلب، يكنى أبا الفضل. كان يتولى الصلاة بسر من رأى، ثم قلد الصلاة ببغداد في جامع دار الخلافة، فأنبأني إبراهيم بن مخلد، قال: أخبرنا إسماعيل بن علي، قال: وقلد الصلاة في مسجد الجامع الذي بحضرة دار الخليفة ببغداد، ويسمى مسجد القصر، أبو الفضل محمد بن هاشم بن القاسم من ولد محمد بن إبراهيم الإمام، وهو والى الصلاة بسر من رأى، فخطب الناس وصلى بهم يوم الجمعة لثنتي عشرة خلت من ربيع الآخر سنة أربع وثلاثين وأربعمائة.
