أبو الفضائل محمد ابن الشيخ أبي الفضل محمد بن عبد الكريم القزويني ساكن بغداد، بها، ودفن بالشونيزية. ومولده في الثاني عشر من رجب سنة ثمان وخمسين وخمس مئة. تفقه ببغداد على الفقيه أبي القاسم يحيى بن علي بن فضلان، وسمع منه، ومن أبي السعادات نصر الله بن عبد الرحمان بن محمد، وأبي القاسم يحيى بن أسعد بن بوش وطبقتهم، وذكر أنه سمع بأصبهان وغيرها من جماعة. وحدث.٢٣٩٥ - وفي سلخ جمادى الأولى توفي الشيخ
