أبو المفضل محمد بن الحسين بن أبي الرضا بن الخصيب بن زيد القرشي الدمشقي، بدمشق، ودفن من الغد بباب الصغير. ومولده في السادس عشر من رجب سنة خمس وعشرين وخمس مئة. سمع من جمال الإسلام أبي الحسن علي بن المسلم السلمي، وأبي الفتح نصر الله بن محمد اللاذقي، وأبي طالب علي بن عبد الرحمان بن محمد بن أبي عقيل. وحدث. ولنا منه إجازة كتب بها إلينا من دمشق في شوال سنة خمس وتسعين وخمس مئة.٨٦٢ - وفي الثالث أو الرابع من المحرم توفي الشيخ المهذب
