أبو طاهر حمزة بن عبد الوهاب بن يحيى بن عبد الواحد بن المسلم بن راشد الكندي الدمشقي. ومولده في الرابع والعشرين من رجب سنة أربع وعشرين وخمس مئة. سمع من أبوي يعلى: حمزة بن أسد بن علي التميمي المعروف بابن القلانسي، وحمزة بن علي بن هبة الله ابن الحبوبي، وأبي القاسم نصر بن أحمد بن مقاتل بن مطكود السوسي.٨٤٨ - وفي سلخ ذي الحجة توفي الشيخ
