أبو المعالي محمد بن علي بن محمد بن الخادم البغدادي البزاز المعروف بابن قشيلة، ببغداد، ودفن من الغد بمقبرة الشونيزي. ومولده سنة خمس عشرة وخمس مئة. سمع من القاضي أبي بكر محمد بن عبد الباقي الأنصاري وأبي الوقت عبد الأول بن عيسى السجزي. وقشيلة: بضم القاف وفتح الشين المعجمة وسكون الياء آخر الحروف وبعد اللام المفتوحة تاء تأنيث.٧٨٢ - وفي الثامن من شهر ربيع الآخر توفي الشيخ
