أبو المفضل محمد ابن الشيخ الأجل أبي العباس هبة الله بن أبي الكرم نصر الله بن أبي الحسن محمد بن محمد بن مخلد الأزدي العدل الواسطي المعروف بابن الجلخت، بواسط، ودفن عند أبيه وجده بمقبرة مسجد زنبور. ومولده سنة اثنتين وعشرين وخمس مئة. سمع من جده أبي الكرم نصر الله، وغيره. وحدث بواسط، وبغداد. وهو من بيت العدالة والتحديث. ووالده أبو العباس أحد العدول بواسط، وسمع بواسط وبغداد من غير واحد، وحدث.٥٥٥ - وفي ليلة عرفة توفي الشيخ الصالح
