محمد بن عثمان بن أحمد ابن عثمان بن هبة الله بن أحمد بن عقيل، الصدر الفاضل الحكيم فتح الدين أبو عبد الله الشيخ جمال الدين بن أبي الحوافر القيسي. سمع من النجيب الحراني " مشيخة " ابن كليب، و" مجالس " ابن مسلمة، و" مجالس " الخلال، وحدث، وكان طبيباً معروفاً بالقاهرة. توفي رحمه الله تعالى في ثاني عشر شهر رمضان سنة ثمان وعشرين وسبع مئة بالقاهرة، ودفن بالقرافة.
