محمد بن محمد بن المسلم بن هلال بن الحسن أبو المفضل (٣) الأزدي الشاهد سمعه أبوه في صغره من أبي الفضل بن الفرات وعبد الكريم بن أبي الفضل الكفرطابي وسهل بن بشر ونصر المقدسي وسمع هو بنفسه فأكثر السماع وحصلت له نسخ كثيرة وكتب حسان مع خلوه من المعرفة سمع منه بعض أصحابنا شيئا يسيرا ولم أسمع منه إلا حكاية واحدة رواها لي عن ابن (٤) الأكفاني وكان مولده عن ما ذكر لي أخوه أبو المكارم عبد الواحد سنة أربع وثمانين وأربعمائة وتوفي وقت صلاة المغرب ليلة الجمعة الخامس أو السادس من صفر سنة سبع وثلاثين وخمسمائة ودفن في داره عند باب الجامع الشامي ثم نقل بعد مدة إلى مقبرة باب الفراديس ٦٩٧٠ -
