محمد بن حمزة بن الحسن بن المفرج أبو عبد الله بن أبي يعلى المعروف بابن أبي جيش (٢) الأزدي الشاهد الشروطي سمع أباه وأبا الحسن علي بن طاهر النحوي وأبا الوحش المقرئ سمع منه بعض كتاب قوت القلوب لأبي طالب المكي عن أبيه مات ليلة الجمعة بعد العشاء الثامن والعشرين من شعبان سنة ستين وخمسمائة دفن في مقبرة باب الفراديس وذكر لي ابنه أبو طالب أنه سأله عن سنة فقال قد بلغت إحدى وسبعين سنة ٦٢٨٣ -
